[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide navigation and housing services to the homeless community to
transition to stable housing with supportive services that outreach, housing and navigation as applicable - detailed statement of all of the programs and services proposed, including conclusions and generalized recommendations
- service delivery model, timeline, and target population for the proposed program component
- Propose overall plan for engaging and enrolling participants in program, provide amount of estimated assistance per individual, duration and estimate amount of rental assistance and wrap around support services per individual
- itemized service budget and budget narrative including all costs and fund sources
- appropriate to the language, culture, and geographic location of eligible participants
- Navigation or Case Management component of the program to the government authority located in Phoenix, AZ.
(2) An optional pre-bid conference will be held on Jun 14, 2018.
(3) All question had been submitted no later than June 18, 2018.
(4) A contract period will be for one year.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
