(1) Vendor needs to provide workers compensation and other medical services to the government authority located in Glendale, AZ.
- Provide experienced staff for treatment of occupational injuries and illnesses, and administration of physical exams.
- willingness to expedite employee and patient appointments to limit loss of time.
- Maintain the capacity to administer and interpret x-rays, audiograms, visual acuity, collection of lab specimens for various analyses including drug screening.
- Maintain accurate medical records and reporting systems for employees receiving medical care necessary to administer the program
- Bill for services rendered in compliance with industrial commission fee schedule.
(2) Contract term will be one year.
