[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide search on-call forensic nursing services as needed, as required, on-call contract(s) with qualified registered nurse(s) or advanced practice registered nurse(s) trained in the field of forensic nursing to provide forensic exams to the victims of sexual assault, sexual abuse or domestic violence to the government authority located in Florence, AZ.
- Work on-call shifts as required by the medical forensic services department.
- Performing comprehensive medical forensic exams including complete medical history, head to toe physical exam and nursing care for victims of domestic violence, sexual assault, and child sexual and physical abuse.
- Collecting, documenting and preserving evidence including potential physical evidence and specimens, photographs as appropriate and maintaining and documenting chain of custody.
- Documentation of patient medical history, including assault history, along with physical exam process and findings maintaining confidentiality of all medical/legal records.
(2) All questions must be submitted no later than November 13, 2018.
(3) Contract period will be for one year.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
