(1) Vendor needs to provide medical, dental and vision insurance services for employee benefits, claims, premium and enrollment are managed, a third-party administrator, fully-insured or level funded program matching their current medical plan, contribution strategy is the same for all eligibility classes, except retiree or COBRA enrollees to the government authority located in Scottsdale, AZ.
(2) All questions must be submitted no later than February 6, 2019.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
