Government Authority located in Glendale, Arizona; USA based organization looking for expert vendor for employee wellness related services.
[A] Budget: Looking for proposal
[B] Scope of Service:
(1) Vendor needs to provide the following employee wellness related services for employees to the government authority located in Glendale, AZ.
a. Willingness to work with website/mobile app, if not chosen for both,
b. Onsite coaching,
c. Individual and group sessions,
d. Sessions vary (e.g. nutrition, financial, stress management, time management, fitness),
e. Live streaming opportunities,
f. Assists with coordination of district-wide wellness functions,
g. Provides various data about participation, interests, etc.,
h. Has wellness background - education and experience,
1. Coordinates with health insurance providers/third-party administrators,
j. Assists with developing and administering employee surveys,
k. Participate in Wellness Committee meetings,
- Interactive Website and Mobile App-
a. Oversees incentives as recommended by Wellness Committee,
i. Including the purchase and distribution of prizes ii. Selecting winners,
b. Participate in Wellness Committee meetings,
c. Coordinates with health insurance providers/third-party administrators,
d. Assist with communication and wellness campaign,
e. Customizable options on Website and Mobile App,
f. Various opportunities to accumulate points,
g. Various opportunities for challenges,
h. Allows for driven point opportunities (e.g. uploading videos, documents, etc.) 1.Verifies completion of certain activities (e.g. doctor visit),
j. Integrates with various devices,
k. System to receive points if attending a particular session,
l. Preferred, but not required, for spouses/family to have access,
m. Willingness to work with consultant/coach, if not chosen for both,
n. Goal setting: individual and group opportunities,
o. Provides advertising of various wellness-related functions in county farmers market, discounts, etc.,
p. Brands and wellness program,
q. Newsletters and other ongoing communication opportunities,
r. Provides various data about panicipation, interests etc.,
s. Assists with coordination of district-wide wellness functions,
t. Ongoing fresh videos and opportunities (rather than have the same videos for five straight years) (latest trends and best practices),
u. Social page for employees to share successes, recipes etc,
v. Assists with developing and administering employee surveys.
(2) All question must be submitted no later than October 3,2019.
(3) A contract term will be for one year.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
