Government Authority located in Phoenix, Arizona; USA based organization looking for expert vendor for athletic trainer services.
[A] Budget: Looking for Proposal
[B] Scope of Service:
Vendor needs to provide athletic trainer services to the government authority located in Phoenix, AZ.
- Athletic trainer shall provide the administrator in charge of athletics and school administration with an accurate record of treatment rendered for all athletic injuries; and facilitate communication between an injured student athlete, head athletic trainer, coaches, parents/guardians and the physician as required or needed,
- Athletic trainer shall evaluate injured student athletes and make prudent medical referrals as a result of each evaluation,
- Athletic trainer shall communicate with medical professional regarding updates on student athletes currently under treatment and approvals for return to play,
- Athletic trainer shall communicate with the school nurse when needed for injury treatment to maintain the relationship between the athletic and academic setting to assist student athletes in succeeding in the classroom,
- Athletic trainer shall maintain medical documentation of student athlete injuries, athletic training room attendance, injury treatments, injury rehabilitation, and physician communication of all injured student athletes,
- Athletic trainer shall communicate daily with coaches regarding player status for practice or game and if applicable per physician recommendation,
- Athletic trainer shall assist the School District as requested in confirming that student athletes have the appropriate medical clearance before returning to sports subsequent to athletic injury or illness.
[C] Eligibility:
- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
