[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide risk factor surveillance system survey services of BRFSS data collection in accordance with protocols established by the CDC. The BRFSS in cooperation with CDC provides data collected from (landline and cell phone) questionnaires of adult population aged eighteen years and older on an annual basis. The survey or data collection services include: combined telephone or cell phone, pre-notifications for both landline and cell phone surveys and The BRFSS survey collects data on health-risk behaviours, chronic conditions, use of preventive services, Health Related Quality of Life (HRQoL), and health-related conditions that are related to the leading causes of death and disability at the state, county, sub-county and national levels to the government authority located in Phoenix, Arizona.
(2) Contract period will be for one year.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
