[B] Scope of Service:
(1) Vendor needs to provide physician services to the government authority located in Phoenix, AZ.
- individual or organization who is, employs, or contracts with individuals duly qualified, licensed and/or certified to provide professional health care services, or who has agents, employees, contractors or subcontractors who are so qualified, licensed, or certified
- provide part-time or full-time physician coverage
- clinical, administrative, and consultation services throughout the department, using a standard of care appropriate for the services being provided.
- Other administrative tasks, such as chart reviews, referrals, review or writing of protocols may be completed offsite and completed at the Contractor’s discretion.
(2) Contract period will be one year.

- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Onsite. Vendor needs to carry work in their office location.
