[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide basic life, basic dependent life, supplemental employee and dependent life and retiree life insurance service to the government authority located in Arizona.
- Designated Account Manager and Account Management Team
- Attend Annual Trust Board Meeting to present Trust-wide utilization report
- Provide a toll-free customer service telephone number
- Maintain a designated customer service and claim processing staff
- Claims forms/submissions
- Correspond with participants if additional information is necessary to complete the processing of claims
- Agree to accept eligibility from each of the Trust employers on a self-administered basis
- Employee Access to Portal for Benefit Summary, pending and paid claims, evidence of insurability
- Employer and/or Designated Consultant access to Claims Data Reporting Portal.
(2) All questions must be submitted no later than October 30, 2020.
[C] Eligibility:
- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
