[A] Budget: Looking for Proposals
[B] Scope of Service:
Vendor needs to provide emergency transportation service to the government authority located in Chandler, AZ.
- Background, experience, financial strength and management: - Provide any and all regulatory investigations, findings, actions, complaints and their respective resolutions. Offeror will specifically include details about any and all emergency (911) contract terminations or contract amendments within the last two years. Additionally, Offeror will detail the circumstances and resolution of any contract disputes or notices of non-compliance
1. Provide a detailed list of all litigation with a reserved value in excess of $100,000 in which the Offeror or a subcontractor is involved.
2. Provide a description of all complaints received by your organization from customers for the period from 01/01/17 to 6/01/20.
3. Provide a description of relevant experience, including contact information and call volume.
4. Provide and document the following:
a. Access to and sources of sufficient capital to provide for implementation and start-up of the contract.
b. Financial reserves or net worth sufficient to sustain the operation in case the proposer has incorrectly estimated expenses or profits from the operation.
c. Any issue or potential event that may have a material bearing on the financial condition, solvency or credit worthiness of the organization. These should include any material contingent liabilities or uninsured potential losses, voluntary or involuntary bankruptcy filings and involuntary contract terminations in other jurisdictions
- Move-up Procedures: -
1. Ensure that Offeror complies with the response time requirements set forth in the Ambulance Contract.
2. Provide that any time the number of dedicated ambulances available for emergency calls in City drops to a total of two dedicated ambulances for the City, Offeror shall immediately move non-dedicated ambulances into the areas of City that lack ambulance coverage; and
3. Provide plan for including MCT and AVL in non-dedicated ambulances and MCT and AVL capacity
- Ambulance Resources Proposed: -
1. Offeror shall include the year, make, model and mileage of each ambulance to be dedicated to service to City including a breakdown between 24-hour and peak-time ambulances being provided.
2. Offeror shall, if applicable, include the year, make, model and mileage of each replacement ambulance proposed. Offeror shall also provide specifications for providing additional compartment space which will support firefighter SCBA and turnouts on dedicated and replacement ambulances to be staffed by City.
3. Offeror shall provide details regarding a maintenance plan to ensure area coverage during times of mechanical downtime. Offeror should detail the procedure for change-out as it relates to mechanical maintenance and repair of ambulances.
4. Provide plan for including MCT and AVL in dedicated ambulances and MCT and AVL capacity.
- Ambulance staffing and method of approach for maintaining All-Hazard Responses within City: -
1. Offeror shall provide the level of City employed paramedics and EMT staffing requested on dedicated 24-hour and peak-time ambulances proposed in its Offer. The response requires the breakdown of staffing between 24-hour and peak-time dedicated ambulances and the designation of the actual peak-time ambulance days and hours of operation rather than just a raw number of hours to be provided per day.
A. Daily peak-time shifts to be staffed with City personnel shall be no shorter than eight hours and no longer than twelve hours. No less than Monday through Friday. No less than one ambulance.
B. The level of City staffing proposed SHALL NOT EXCEED the available staff resources for 24-hour and peak-time ambulance staffing set forth in Section 3 above.
2. Offeror shall provide documentation demonstrating how its proposed ambulance staffing satisfies the objectives set forth in Section 2 above.
- A. To maintain “all-hazard” response units available within City. An all-hazard response unit is an ambulance that can not only handle all types of medical incidents, but can also respond to some fire situations, initiate the Incident Command System, perform forcible extrication of persons who are trapped, stabilize hazardous materials incidents or other, similar, emergency situations not primarily involving the medical transport of a patient that may arise within City. Each all-hazard response unit would be staffed with two personnel who have medical certifications. The firefighter certifications, hazardous materials training and forcible extrication training required for all-hazard units would be met by the firefighter paramedic. This unit would also carry personal protective equipment for the firefighter paramedic, including turnouts and a self-contained breathing apparatus (SCBA)

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
