[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide physical and mental health wellness screening services for all non-probationary employees of the police department to the government authority located in Tucson, AZ.
- These Wellness Screenings shall be part of a police department focus of overall employee wellness and are prioritized to enhance employee physical and mental wellness and reduce risks for job related injuries and illness
- Any associated costs will be the responsibility of the contractor
- Contractor is responsible for records management, data entry and medical transcriptions
- The contractor shall establish and maintain procedures and controls for the purpose of assuring that personal identifying information or protected health information contained in records or obtained from the employee or others shall not be used by or disclosed to any unauthorized persons
- At the conclusion of the screening, the Provider who performed the screening and reviewed the results will meet with the employee one-on-one and will discuss the findings and recommendations with the police department employee
(2) A pre-proposal conference will be held on January 21, 2021.
