[A] Budget: Looking for Proposals
[B] Scope of Service:
Vendor needs to provide HIV rapid test kits supplies on an as needed basis to the government authority located in Phoenix, AZ.
- Provide a comprehensive selection of materials and pricing through established catalogs and brochures, if applicable;
- Catalog and brochure price lists shall be available in electronic (web-based) and/or hardcopy formats;
- All electronic and hardcopy catalog and brochure price lists shall contain the same products and pricing,
- All costs associated with creation, distribution, management, set up, interface, and delivery of all catalog and brochure price lists shall be borne by the Contractor,
- HIV Rapid (point-of-care) tests that have been approved by the Food and Drug Administration (FDA), for a Clinical Laboratory Improvement Amendment (CLIA) waived tests. CMS.gov CLIA Information:
- The rapid test kits shall be capable of detecting HIV-1 p24 antigen and HIV-1 and HIV-2 antibodies (Ag/Ab Combo);
- The rapid test kits shall detect HIV-1 p-24Ag in less than thirty (30) days from infection; and
- The rapid test kits shall have a minimum shelf life of fourteen (14) months,
- Training materials and procedural instructions for proper handling and administration of the rapid test kits.
