[A] Budget: Looking for Proposals
[B] Scope of Service:
Vendor needs to provide registered dietitian consultant services on an as needed basis to the government authority located in Phoenix, AZ.
- Provide staff that meet the requirement of 42 CFR § 483.60(a).
- Provide community-based nutritional education, nutritional analysis, menu modification, analysis, counseling and education.
- Preparing and modifying materials used for nutritional education.
- Recommending for purchase or acquisition nutritional education materials for Veteran/Members and/or staff education.
- Review proposed menus for balance and physical nutritional adequacy. Make recommendations and adjustments in agreement with the food Manager.
- Provide Nutritional Analysis for menu cycle with weekly summaries.
- Create a diet and diet extensions for medical ordered diets. Agency Medical Director shall approve the suggested medical diet.
- Consult with physicians and nursing personnel to determine and meet nutritional needs and diet restrictions of Veteran/Member.
- Audits on a regular basis to ensure agency Homes operate in accordance with health and safety codes.
- Recognizes, evaluates and addresses the nutritional needs of every Veteran/Members.
- Participate in Quality Assurance program.
- Have a Dietitian at each home as needed per the agency Home Administrator.
