Vendor needs to provide Non- Medical Transportation Broker to arrange for Non-Medical Transportation services for transportation services for adults, children and families whom may include individuals with disabilities who receive services,
- broker is defined as an entity that has agreements with multiple transportation providers across the state to meet the non-medical transportation needs services include transportation of clients to and from:
- Supervised Visitations, Family Visits, Transitional Visits, Court appointments, Employment, etc.
- All medical transportation (i.e. transportation to behavioural health appointments, medical and dental appointments, to pick up prescription, lab appointments, etc.),
- To provide a non-emergency transportation broker service to ensure reasonable access to and from the services needed,
- Foster care children are not often placed in close proximity to their parents, and professional service providers, etc. to the government authority located in Phoenix, AZ.
[C] Eligibility:
Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
